RetinaRest

· Payment of rental / shipping fees must be made prior to dispatch of equipment.

· The customer is responsible for ensuring that RetinaRest receives all of the contracted rental equipment within 4 business days after the agreed upon rental period. Failure to return equipment on time will incur fees equivalent to the weekly rental fee for each partial week the equipment is not returned.

· All units must be returned to RetinaRest in good condition, allowing for normal wear and tear.

· If equipment is not returned to RetinaRest in good condition, allowing for normal wear and tear, the customer will be charged the purchase price of the equipment.

· RetinaRest reserves the right to issue a $100.00 handling charge for orders that are cancelled after they have been shipped. 

· RetinaRest makes no medical representations regarding the use of the equipment.

· RetinaRest is not responsible for any medical condition arising or resulting from any medical treatment including, without limitation, eye surgery rendered to the customer by a physician or qualified healthcare provider.

· RetinaRest is not responsible for any injuries/damages resulting from use of the equipment in a manner not intended.

· RetinaRest advices customers to consult their ophthalmologist prior to use of the equipment.

· Placement of an order signifies acceptance of the Rental Terms and conditions.
